C.A.R.E. (Community Assisted Response for Emergencies) Registry is an initiative created to better assist our citizens with Alzheimer’s Disease, Cognitive disorders, Autism, or other special needs. The Registry is a voluntary program designed to help emergency first responders help those who need extra assistance.
Sheriff Doug Adams is excited to announce the commencement of our C.A.R.E. Registry program. C.A.R.E. stands for Community Assistance and Response for Emergencies and is a voluntary program where individuals, families, or caregivers can register those who might be vulnerable, at-risk, or have special needs living within Pottawatomie County with the Sheriff’s Office. Though primarily designed to provide awareness and tools for deputies interacting with individuals with special needs, we believe by broadening the scope, we can also help those with a variety of needs.

Individuals or their designated guardian, can submit an application by contacting our office at 785-457-3353 or via email at care@ptsheriff.com. The information to be entered in our database will be kept confidential. Information in the database includes personal contact information, caregiver contact details, a current photo, needs unique to the individual.

How it works: When a Deputy encounters an individual with unique needs, the Deputy can access the database to quickly get information about the individual, which can include how to contact their caregiver.

C.A.R.E. participants are able to provide comprehensive information including communication preferences, triggers, special concerns, and calming strategies along with a photograph and emergency contact information. The Sheriff’s Office will build and maintain this confidential information database. Sheriff’s Office staff can access this secure information prior to or during a contact so they are better equipped for interactions, can de-escalate situations, and are able to earn the trust of those who might require or prefer different styles and approaches.
If you or someone you know would benefit from this service, please fill out the C.A.R.E. registration form below.  Please note that once a registration form is submitted, the system will send an automated email back to the email address entered.  A follow up meeting will be completed by staff shortly thereafter. Once your C.A.R.E. registration information has been added to our system, we will send you a letter confirming your participation as well as C.A.R.E. stickers for your vehicle and/or residence.  Adult individuals must consent to join the program themselves, unless a legal guardian with decision-making authority enrolls them.

If you have any questions about the C.A.R.E. Registry program, please us at care@ptsheriff.com.
